During JND Healthcare Management’s Triage and telephonic case management phase it sometimes becomes necessary to refer a case out to field case management or task assignment. JND Healthcare is contracted with National Field Case Managers that service all 50 states.

When cases are transitioned for on-site intervention, our nurses collaborate with Field Case Managers. We expedite our field activities through the immediate sharing of all available medical and demographic information, including a summary of case management services to date. This integration of our services maintains case momentum and guarantees a consistent strategy throughout the continuum of care.
